A 48 year old woman presents with dysphagia. This is both to liquids and solids. She has also associated retrosternal pain and she does sometimes regurgitate food. She has not had any weight loss. There is no past medical history and she is well otherwise. There is nil of note on examination and bloods are normal. What is the most likely diagnosis?
